Thank you for using MyHealthChart to obtain certain records from your hospital stay. Please note that within 36 hours, your information about the care you received while in the hospital will be posted.

If you have an issue with the content of the information on your medical record, please contact your primary care physician or specialist directly.

Access Issues
To access MyHealthChart, you will first need a Validation Key Code to set up your account. Then you will need to set up your username and password.

Crouse patients to request your Validation Key Code, visit http://www.crouse.org/mychartsupport/default.aspx or call 315-470-7675 Monday through Friday, 7:30 a.m - 4:00 p.m.

Community Memorial patients to request your Validation Key Code, visit http://www.communitymemorial.org/my-health-chart-support or call 315-824-6580 Monday through Friday, 8:00 am - 4:00 pm.

If you have forgotten your password, enter your username and click the ‘Forgot Your Password’ link on the logon screen.
